CASE STUDY 1 – ANGELA Consultation with Dermal Therapist / Cosmetic Nurse
Angela is a 45-year-old Caucasian woman interested in a surgical procedure to treat jowling and a double chin. She feels she has lost the definition of her jaw line and is developing marionette lines. On observation, her skin has lost elasticity, though this seems only apparent on the lower areas of her face. The sub-mental area presents a small pocket of fat though no excess skin. The recommended surgical procedures were discussed and the patient received supporting information. Angela is booked for a consultation with the doctor in one week’s time to clarify the suitability of the discussed procedures.
Suitable surgical Options for ANGELA
The sub-mental area presents a small pocket of fat will require liposuction procedure on Angela’s neck and jowls. Platysmorrhaphy procedure will ensure tightening of the muscles and the removal of fat from under the platsma muscles (McCarty 2005, pg.305).
Expected Post-operative Results
A facelift operation will enable Angela to re- define her jawline, reduce sagging, tighten skin around the lower areas of the mouth, and removal of small pockets of fat in the submental areas. Tightening of the skin around the neck will ensure that the folds present under the corners of her mouth are improved (McCarty 2005, pg.305).
Surgical Approach and Technique and Anesthesia used
A skin only facelift surgery will be required to remove the small pockets of fat around the jowl area. The surgeon will remove the unwanted fat deposits and trim away the excessive skin. In the case of double chin, lipolysis injection over a series of sessions will yield significant results. The most appropriate technique for facelift technique of the jowl is the vertical facelift (McCarty 2005, pg.306).

A facelift operation can be performed by a surgeon as an outpatient procedure with very minimal downtime (McCarty 2005, pg.307).

Post-operative garments will be worn around her chin and neck for a period of time by the doctor’s appointment will be set up to assess the recovery process and handles any post-operative issues (McCarty 2005, pg.307).
Scarring
The incision scars will become permanent but will significantly fade and improve overtime (McCarty 2005, pg.308).
Pre/ Post-operative Care
Prior to the surgery Angela is expected to maintain a healthy diet as proper diet will aid in the recovery process, Angela will experience bruising, discoloration of the skin, and swelling, during the early phase of recovery (McCarty 2005, pg.309).
Complementary Dermal Therapies
Follow up appointments and therapy sessions will be set up as Angela is concerned about re- define her jawline, reduce sagging, tighten skin around the lower areas of the mouth, and removal of small pockets of fat in the submental areas. Therapy will help address this concerns and compliment facelift surgery (McCarty 2005, pg.309).
CASE STUDY 2 – GEOFF
Consultation with Dermal Therapist / Cosmetic Nurse Geoff is interested in finding out some information about rhinoplasty. He has been considering the procedure for a number of years as he views his face as being out of proportion. With observing Geoff’s facial angles it appears the dorsum has a visible hump and the naso-labial angle is low making the nose appear hooked. Also, the chin is slightly receded causing the nose to appear larger than it really is. We discussed the operation(s) and viewed some pre-post photos. Geoff has received information and is booked to see the doctor for a consultation.
Suitable Surgical Options for GEOFF
By observing Geoff’s facial angles it appears the dorsum has a visible hump and the naso-labial angle is low making the nose appear hooked. Also, the chin is slightly receded causing the nose to appear larger than it really is rhinoplasty surgery is recommended to solve the issue of facial imbalance (Daniel 2004, pg.2156).
Expected Post-operative Results
The expected results after the surgery will include straightening of the crooked nose to complement other facial features, elimination of unevenness or visible bumps, and the correction of naso- labial angle (Daniel 2004, pg.2156).
Surgical Approach and Technique and Anesthesia Use
Rhinoplasty surgery takes about 2 to 4 fours for the operation to be complete. General anesthesia or IV sedation will be used during surgery. Tiny incisions will be made to remove the graft tissues and the incisions are then closed. Rhinoplasty technique can either be closed or open (Daniel 2004, pg.2158).
The Hospital/Clinic Stay and expected Downtime
Rhinoplasty surgery is generally an outpatient procedure and Geoff will be discharged an hour later after the operation. Geoff can resume work after two weeks of his surgery (Daniel 2004, pg.2159).
