Treatment, Program and Discharge Plans

T​‌‍‍‍‌‍‍‌‍‌‌‍‍‍‌‍‌‌‌‍​ask: Each student will develop an initial treatment plan using the data gathered during the case study assignment, placing the client into appropriate Recreational Therapy programs at a hypothetical facility. Students will develop goals and objectives, and interventions for the client based on focus areas identified in the assessment. Then they will create one Program Plan for a specific intervention they have recommended for the client. As part of this assignment, students will also verbally report client information in a mock treatment team setting. Your treatment plan will be evaluated based on: appropriateness/soundness of goals, objectives and interventions appropriateness/soundness of assigned programs (descriptive titles) completeness and technical accuracy of documentation reality as much as possible easy to follow format After completing the Treatment and Program Plans, each student will gather as many resources as possible appropriate to the client. Then, using the internet, students will research specific resources to assist their client in reaching their ultimate leisure goals post discharge. Part One: Complete Treatment Plan Develop an initial treatment plan from data collected during the case study assignment, placing the client into available RT programs at a hypothetical facility. First create a Case Study Summary (demographic overview). Practice your clinical language utilizing as much vocabulary and as many acronyms as possible. Be sure to know the meaning of the abbreviations, as you will be reporting verbally in a mock treatment team setting. Example: Pt is African American, 42-year-old female. Married with 3 children, ages 3,5,7; is one of seven siblings; mother living. Admitted with dx of severe depression secondary to tbi due to a mva. Client also has COPD; smoker for 20+ years. Previously employed as a secretary for large insurance company x15 years; family financially secure. Mostly sedentary lifestyle. Admitted to facility last Saturday. Prior interests include bridge, camping, ceramics, singing, spectator sports and television. ELS- two weeks). Next select two domains discussed in class to address in treatment. These domains should reflect information given during the assessment portion of the case study. Do NOT randomly choose domains/goals/objectives/interventions. These items should be individualized to the client based on need. Type your selected goals and objectives beneath your case study summary. Be sure to include frequency and length of time for each objective. You must also state specifically what Recreational Therapy program you recommend in each objective (interventions). These are hypothetical program names you need to create. Part Two: Complete Program Plan You will choose one hypothetical program name from Part 1 and develop this program into a one-page program plan. This is not a specific structured activity but rather a broad-based program name. (i.e., Creative Self-Expression, Anger Management, Leisure Education, Physical Wellness, Healthy Lifestyles, Stress Management etc.). Include the following elements: Name/Type of Agency: (This is hypothetical) Name of specific program (i.e., Creative Self-Expression, Anger Management, Leisure Education, Physical Wellness, Healthy Lifestyles, Stress Management etc.) Program description- this is a broad overview describing the program Create 2 goals of the program (these should be based o​‌‍‍‍‌‍‍‌‍‌‌‍‍‍‌‍‌‌‌‍​n information from the textbook and above tx plan assign.) Develop 2 expected outcomes/objectives per goal clients may experience from participation in this program Part Three: Treatment Team Meeting 1 In class, you will present your client and the program plan you have developed for them to a mock treatment team. You will need to articulate clearly why you chose particular goals and objectives as well as the type or program and expected outcomes. Each member of the treatment team will have a peer-review form they will complete at the end of all treatment team presentations. Your treatment team will be randomly allocated in class and you will also peer-review others. Part Four: Discharge Plan Here you are now assuming that the client went through your Treatment Plan, has completed the Program and now is ready for discharge. You will then provide a brief summary of the location to which the client will be discharging. Based on information from the Case Study and Treatment Plan, research a minimum of 5 different resources that the client can access post discharge. Resources can be chosen from the following topic areas (DO NOT REPEAT AN AREA): adaptive/inclusive recreation programs transportation social opportunities physical activities adaptive equipment job placement/vocational training independent leisure activities support groups communication support advocate services community based programs local parks and recreation volunteer opportunities library information adult education courses community art /theater local community colleges. Include specific information for each resource with as much detail as possible (i.e., description of what the resource offers, prices, phone numbers, websites, locations etc.). Resources must be based on information gathered during the case study and treatment plan and individualized to the client (i.e., do not simply provide a list of general support groups; instead, find a specific group to TBI if the client has a brain injury). Part Five: Discharge Summary Often time Discharge plans are multi-disciplinary in nature with only a small amount of space provided for each discipline. Use the provided Discharge Summary Form, completing the form based on your Case Study and Treatment Plan. The purpose of this is to practice summarizing information while continuing to maintain the intent of the document. Part Six: Treatment Team Meeting 2 In class, you will present your client and the discharge plan you have developed for them to a mock treatment team. You will need to articulate clearly why you chose particular resources as well as how you feel these will support the client in transition to the community. Each member of the treatment team will have a peer-review form they will complete at the end of all treatment team presentations. Your treatment team will be randomly allocated in class and you will also peer-review others. Submission You will submit via Turnitin a report containing a Case Study Summary with selected goals and objectives (including frequency and length of time for each objective, and the recreational therapy program you recommend for each objective) (Part 1), a Program Plan (Part 2), and a Discharge Plan (Part 4). To the report, you will attach a copy of the Discharge Summary Form (Part 5), as well as copies of completed peer review sheets from your treatment team members (Parts 3 and 6 – not included in word coun​‌‍‍‍‌‍‍‌‍‌‌‍‍‍‌‍‌‌‌‍​t).
