Some up with a scenario that centers around a fictitious patient that you will be treating. You must use the course resources to indicate your patients: initials, social history and health history (include the disease/ disorder). Use the guidelines below to help you along your way.

Part 1 Project Guidelines:

For this part of your project you will write a report to discuss the following information:

Patient Initials – Do not provide your patient’s name because you will want to maintain patient confidentiality, but use his or her initials.
Patient Social History – Write at least one paragraph to discuss your patient’s social history in terms of the following:
Family life
Occupation
Recreational aspects of their personal life
Health History – Describe the health history of the patient in at least 200 words. Include the following information:
Diagnosis – Discuss the diagnosis of your patient in detail.
Patient’s chief complaint
History of present illness
Allergies
Medications they are taking
Past medical history
Past surgical history
Psychiatric history
Family history of illness
