Title Page – This will be your cover page for the report.
Table of Contents – This will be a separate page.
Table of figures (If Any) – This will be a separate page.
Transmittal Letter – This will be a separate one-page document.
Executive Summary – Should be no longer than one page.

Introduction
In the actual final report, the introduction includes details about the question or problem the report will answer, identifies the primary audience for the report, summarizes the scope and limitations of the research, and gives a general overview of where the information comes from.

Findings
In the actual final report, the findings section is the most detailed part of your report and has the supporting information needed to justify your summary, conclusions, and/or recommendations. This will include, but not be limited to, the information from your Annotated List of References, your interviews/surveys, and all other research.

Analysis/Summary
In the actual final report, the summary reviews the main points presented in the body and may include conclusions and recommendations.
It provides a final comment or conclusion to the report.

Appendix/Attachments
In the actual final report, at the end,
The appendix must include
List of References (minimum of five sources)
List of the questions you asked when you conducted an interview
AND/OR
Copy of a survey you administered as part of your research and a tally of the results of the survey
The appendix may also include
Charts, tables, photos, maps, or other graphics not included in the body of the report
Any other items you feel are necessary to support your analysis, but which don’t fit into the body of the report.
