Your self-care plan can be in any format you choose (Samples are included, but you can be as creative as you want, don’t feel tied to those formats).
Plans need to address the following elements:
Mind
Body
Emotions
Spirit
Work
Relationships
Other (if needed)
Potential Barriers and how to overcome
Be sure to describe the “what”, “when” and “how” of your self-care plan
Cite in APA format
