Practicum Hours Description During my 25-50 hours of practicum, I engaged in a mix of practice setting and non-practice setting activities that enriched my learning experience. In the practice setting, I dedicated approximately 20 hours to direct patient care, where I assisted in health assessments and developed care plans for individuals with chronic health conditions. These hands-on experiences allowed me to apply theoretical knowledge and improve my clinical skills. In addition to my practice setting activities, I participated in several non-practice activities. I attended two DNP student presentations (4 hours), where I learned about innovative practices in nursing and healthcare delivery. Furthermore, I attended a professional conference (6 hours) focused on advancing nursing practices and health policy discussions. I also engaged in a grant writing workshop (2 hours), which provided insights into securing funding for healthcare initiatives. Lastly, I participated in the Virtual Practicum Work Room (3 hours), where I collaborated with peers to discuss challenges and share resources related to our projects. DNP Essentials and Accomplishments One significant intervention that represents an accomplishment towards meeting DNP Essential II (Organizational and Systems Leadership for Quality Improvement and Systems Thinking) was my involvement in developing a quality improvement initiative aimed at enhancing patient education on chronic disease management. During my practicum, I conducted a needs assessment and created educational materials tailored to patients’ specific health conditions. This initiative not only improved patient understanding of their conditions but also demonstrated how I have enhanced my activities based on my DNP learning. My coursework emphasized the importance of evidence-based practice, which I applied when designing the educational materials. Progress Towards DNP Project Throughout the practicum, I made strides toward my DNP project focused on improving mental health resources in primary care settings. I conducted interviews with primary care providers to identify barriers to addressing mental health issues within their practices, which will inform the development of an integrated care model. Additionally, I gathered data on patient outcomes related to mental health screenings, which will be crucial for demonstrating the effectiveness of my proposed interventions. Collaboration with Healthcare Team Collaboration with other members of the healthcare team was a pivotal aspect of my practicum experience. I worked closely with nurses, social workers, and physicians to coordinate care for patients with complex needs. For instance, during interdisciplinary team meetings, we discussed patient cases and shared insights on best practices for managing chronic diseases. This collaboration not only fostered a holistic approach to patient care but also emphasized the importance of teamwork in achieving better health outcomes. Learning Gaps and Future Plans While my practicum experience was enriching, I identified some learning gaps regarding advanced data analysis techniques, particularly in evaluating health outcomes data. To address this gap, I plan to enroll in a workshop focused on statistical methods for healthcare research. Additionally, I will seek mentorship from faculty who specialize in data analysis to gain practical insights and support as I progress through my practicum. This proactive approach will help ensure that I can effectively analyze data relevant to my DNP project and contribute to evidence-based solutions in healthcare.
