    developing/implementing this application a success? (Note: these questions
    are about organizational culture and process quality- not about results)
    Please assess this by addressing the following:
    2.1 In general, do the organization’s development/implementation staff have to comply with
    governance requirements and/or are they bound by organizational SOPs (standard operating
    procedures) & hierarchy?
    2.2. Do the IT development/Implementation staff appear to be reactive or proactive? Do they follow
    long term plans or shorter-term backlogs (lists) of current challenges? Do they seem to be “fighting
    fires” a lot?
    2.3. How does the organization itself measure success in respect to information systems
    development/implementation activities?
    2.4. By that measurement, were these activities (as you studied them) successful?
    2.5. As they worked on implementing the application, did the organization’s
    development/implementation team(s) members encounter changing business requirements? How
    did they respond to these changes and how nimble were they in doing so?
    2.6. Did the design, acquisition/development/outsourcing processes work? Where were they
    flawed? Were established procedures followed? Were new procedures defined? In hindsight, could
    the development/acquisition/outsourcing activities have been done better or that should be
    corrected?
    2.7. Did the organization's (users or IT) staff pick up new skills during this application
    implementation? What were they?
    2.8. What improvements could/should the organization adopt for information systems
    implementation in the future (i.e. what larger lessons have been learned by you)?
