Occupational therapy; empathy case study

AnitaHanke is an 82 y.o. female admitted to your SNF for STR 5 weeks ago due to a recent R THA. She

had planned for this hip surgery for several months because of “excruciating” pain and was anxious to

leave for home after OT/PT for 1 week because she is her husband’s caregiver, as her husband is in early

stages of AD. They have been happily married for 62 years and had never been apart until this

hospitalization. Currently, her daughter is caring for her husband while she is recovering.
But on her 5th day at the skilled nursing facility (SNF), Ms. Hanke demo signs and symptoms (s/s) with

complaints of (c/o) shortness of breath (SOB) and oxygen saturation (SpO2) 85% sat on room air (RA). Pt

was sent to acute with diagnosis (dx) pneumonia (PNA) due to COVID-19 infection. After being on a

ventilator for 8 days, she was sent back to your facility in the COVID-19 unit to recover. At your 100-bed

facility, there has been COVID-19 outbreak. All nursing homes in the state are under lockdown, in which

no visitors are allowed to enter. Thirty-two pts had acquired this nosocomial infection, with four pts who

have expired, one of whom was Mrs. Hanke’s roommate, Ms. Claire Annette. Mrs. Hanke and Ms. Annette

had become fast friends because of their common interest in rescuing dogs. After 4 days of being in the

COVID-19 unit receiving no therapy, she is transferred to the general population to continue with therapy

after 3 consecutive days of negative tests for COVID-19.
On the first day since she has resumed her therapy, she demo being upset. “I miss Iona (her rescue dog).

I can’t believe Claire (Ms. Annette) is gone. And I just got a call from Manny (her husband). He believes

this virus is not as bad and wants to see me today. He just doesn’t understand.” Mrs. Hanke is refusing

therapy. Demonstrating positive interpersonal skills including, but not limited to cooperation, flexibility,

tact, and empathy, persuade Mrs. Hanke to participate in OT tx to meet her LTG of discharge (D/C) home.
