Nursing in the Community

From the suggested reading materials, select a home visiting or case management program and summarize in 250 words or less the program goals and objectives and what interventions are provided. Focus on nursing activities when possible. Develop a logic model that would reflect that program. Any format is acceptable as long as the critical elements of inputs, outputs, outcomes or impacts are included. Module 8 will require the student to include a logic model so choosing a program that is of personal interest and lend itself to the final module is encouraged. There will be aspects of the logic model that will require research so references are required using APA style. Place the detail references on a separate page with the logic model. The assignment should be presented in APA format in a scholarly essay of 1500 to 2000 words. At least two scholarly sources, other than the textbook and provided materials should be utilized.
Goals
After completing this module, you will be able to do the following:
• Describe four planning models that promote community engagement in population health program planning.
• Discuss standards and principles of the case management practice making sure to delineate the principles of case management.
• Assess the need for case management and factors influencing the case management situation.
• Describe the competencies of effective case managers.
• Discuss case management as it is used at both the individual and population levels. Delineate examples of each.
• Summarize the advantages and disadvantages of a home visit as a means of providing nursing care.
• Distinguish the relationship between home health nursing and population health nursing.
• Analyze the elements of a program theory called the logic model and its usefulness in public health program planning.
• Analyze the elements of the program theory called the ecological model and its usefulness in public health program planning.
• Outline the steps in the process of evaluating a health-related community-based program.
