Mediation Evaluation Form

Imagine that you run a community mediation program. Create an Evaluation Form that the parties will complete following their sessions to help you determine the success of your program and mediators. You may search the web for other evaluation forms used by programs that use facilitative mediation as an example for your own work; however, as per WilmU’s Academic Integrity Policy, you are expected to create your own form. If you do use a sample, make sure to cite it as a resource.
