Research and analyze four insurance plans offered by a federally
facilitated health insurance exchange (marketplace). Download the Excel “Health Insurance
Exchange Assignment Template” which includes a completed submission example on one sheet
and a blank template for you to complete.
Part One: Pick a zip code
Pick a non-Florida zip code for a state with a federally-facilitated exchange. The list of
states below has federally-facilitated exchanges.

Part Two: Pick Your Four Plans
Use the website: https://www.healthcare.gov/see-plans/#/ to find all available insurance
product offerings for your selected zip code. To find products, input your zip code. Then
hit "skip" until you see "Go to Full-Price Plans." (Close the pop-up that says, "Help
Comparing Plans.")
Next, pick at least four insurance product offerings, including one from each metal level
(bronze, silver, gold, and platinum). If a platinum plan is not available for your zip code,
you may substitute another metal. Some states do not have platinum plans.
Part Three: Complete the Template
After you select four insurance products from any insurance company, fill out the Excel
template titled “Health Insurance Exchange Assignment Template.” When inputting
information in the template, make sure you specify the zip code you used (at the top)
and the metal type for each plan.

    • Platinum (or Substitute): Select a platinum plan if available. If not, choose a different metal level (like a high silver or low gold) to complete the four plan selection.

Part Three: Completing the Template

  1. Download the Template: Download the "Health Insurance Exchange Assignment Template" from the provided link.

  2. Enter Zip Code: At the top of the template, enter the zip code you used for the search.

  3. Fill Out the Template: Carefully input the information for each of your four plans, including:

    • Metal Level: Bronze, Silver, Gold, or Platinum (or substitute).

    • Plan Name: The specific name of the insurance plan (e.g., Blue Cross Blue Shield PPO).

    • Monthly Premium: The cost of the plan per month.

    • Deductible: The amount you have to pay before the plan starts covering costs.

    • Co-pay: The fixed amount you pay for doctor visits, prescriptions, etc.

    • Co-insurance: The percentage of costs you pay after your deductible is met.

    • Out-of-Pocket Maximum: The maximum amount you'll pay out-of-pocket in a year, after which the plan covers 100%.

    • Network: The types of healthcare providers and hospitals covered by the plan (e.g., HMO, PPO, POS).

Remember:

  • Be sure to save your completed template and label it appropriately with your zip code.
