Genogram includes at least two individuals from each of three generations, the participant and siblings, his or her parents or their siblings and his or her grandparents; may include participant’s children
Ages, dates of birth and of death, family structure (divorces, marriages, adoptions), cause of death and any health conditions are included for each individual on the genogram if known
Pathophysiologic paper one to two pages page plus references and to include
