Instructions
Use the following scenario as the basis for this assignment: Developing Care team as a Case management Director known for thinking outside he box and comin up with fresh ideas.
For the last several years patient outcomes have declined, readmissions have increased, and several safety issues have ben brought to your attention by the clinical staff. Your first project is to create a proposal that will help improve these numbers.
The team members you can get some input from are :
- Registered Nurse – Director of patient safety and Quality.
His Input :
There is always the possibility this could go poorly. Possible risks to consider, which can be monitored and mitigated are:
a. Communication: a leader must be clearly responsible for communicating expectations and results with the team.
b. Financial: Reassigning of hiring new staff incurs an expense, nd if the pilot is not implemented permanently, you will need to layoff or reassign your team to potentially lesser-paid roles,
c, Operational: We really need to track outcomes for this pilot. Our data needs to be captured clean and quick. What timeline are you looking at for this implementation?
d. Shared governance: This pilot has the potential to support our organization’s culture of collaboration. Make sure you consider ways to solicit input from key stakeholders at multiple levels of seniority and cultural care.
The data that will help support this are:
- As Accountable Care Organization (ACO) we already track the 30+ measures which we also report to CMS. We can easily pull data from any of these measures to align with the CCT proposal needs.
- Aligning the CCT goals with our ACO quality measures is also a sound strategy to gain Board of Director’s approval of a formal CCT policy.
Development of a question or 2 to add to our discharge survey will help capture patient experience with the CCT.
Other staff that can give input to this proposal are. - Director of clinical Operations MD
- Chief of surgery, Director of Rehabilitation, Chief Nursing Officer (oversees case mangers), and Director of admissions. Each has a key role to play in the patient’s experience and journey through our facility.

Questions 1 ;
- Can I reassign staff ?
answer: Yes you can reassign staff and the following procedure is recommended;
-1. Meet with the potential team members department supervisor. Provide them with an outline of the CCT proposal explaining your rationale for needing their staff to work in a different capacity. Ask for their recommendations on who might be best-suited for the CCT role(s). - Meet individually with the staff recommended by the department supervisor. Conduct an interview process just as you would for hiring into a new role.
3.. Assess candidates and make job offers within two weeks. Contact Human resources to process the necessary paperwork for department transfer/reassignment, and any compensation changes.
Question 2. Can I hire new staff?
Answer: Yes, we do have budgetary support to hire new staff. ideally, we would like to keep new hires to a minimum and utilize internal talent whenever possible. This both reduces our expenses and promotes our hospital culture.
Question 3, What is a realistic timeline for the HR side of implementing this proposal?
Answer; I would estimate:
1 week for completing with current managers
2 weeks for interviews/candidate selection
2 weeks for follow-up, extending and accepting offers.
Once you identify candidates, any salary questions can be sent to HR directly. They will get them answered ASAP so you can proceed with extending an official offer.
You will develop a proposal for review by the hospital leadership team. This proposal should contain the following (at minimum):
Executive summary detailing the need for collaborative care teams (CCT) and why they will benefit the hospital.
At least one paragraph to summarize each of the following components, citing relevant research as applicable. Assume fictional information to formulate your rationale:
Benefits of CCT to patients
Benefits of CCT to clinical staff
Structure of CCT (what providers are involved, and to what degree)
Oversight and assessment (who is responsible for tracking the success or failure)
Operational/Financial risks or areas of concern
