Clinical Quality Improvement/Measurement Tool

A 10 page paper excluding title page and reference list, written in APA 7th edition format. The student will address how a case scenario important to the provision of quality patient care may be resolved for an entire organization. Students may choose from the provided case scenario or create a similar scenario based upon work completed during content analysis and the matrices assignment.

The student will select at least two patient population measurement tools and compare and contrast appropriateness to the issue. The student must choose which patient population measurement tool is most appropriate and address the tool’s relationship to the identified scenario and how the tool can be utilized in addressing the identified scenario.

***I chose inpatient falls for this assignment
***The measurement tools I chose is Morse Fall Scale vs Johns Hopkins Fall Risk Assessment Tools.

Furthermore, the student must propose a quality improvement plan, utilizing a decision tree-logic model.
This paper will be based on a specific scenario or “hypothetical” case evidencing a need for organizational leadership systems for organizational issues or patient populations, health
outcomes issue (for the whole organization).

  1. Address which problem the paper will seek to tackle (Inpatient falls). Provide a background of the issue, addressing why the scenario presents a problem requiring intervention. Include literature review addressing at least 5 current peer reviewed scholarly articles pertaining to the clinical issue and how other organizations have resolved the issue. The literature review should discuss and analyze current evidence regarding the use of interventions targeting the issue. May also address information regarding your identified clinical issue from appropriate organizations such as WHO, CDC, CMS, NIH, etc. Identification and reporting of the setting, issues to be resolved based on an assessment of the need. Identify the quality improvement process to be utilized and address specifically how the different steps of the process will be addressed in the proposed project.
    1. Identify the appropriate tools (patient population outcome tools). Should compare and contrast at least 2 different measurement tools that might apply. Include the aim of the outcome measurement and description of each tool selected. Review the weight/quality of the evidence found in the
articles describing the patient population tools. Identify which tool should be selected as the most reliable for your given measurement.
  • Here, I choose the Morse Fall Scale and The Johns Hopkins Fall Risk Assessment Tool

  1. Describe the measurement design procedure and justification of the rigor of measurement based on validity and reliability of the selected patient population tool (critique how validity and reliability were verified before previous use by the authors, if applicable, or how you will ensure that it measures what it should measure).

    1. Discuss the process of data collection and how this patient population tool should be implemented in your clinical setting. Describe additional potential issues of data collection that might impact the validity and reliability of the data. Rank the usability of the tool as it relates to other available tools for your intended purposes (e.g., does the tool contain too many questions to logically be acceptable in primary care?) Show how the
results should be monitored, displayed and benchmarked on an on-going basis. (May include in body of paper or in appendices).

  2. Identify potential interventions based on literature findings of improvement for similar case resolutions that the nurse leader should expect to implement to impact positive change and to improve the care processes.

  3. Identify and discuss implications for evidence-based practice and nursing research and how nurse leadership may be effective in this practice setting.
