Each question is worth a maximum of 10 points, spelling & grammar are worth an additional 10 points, for a total of 100 possible points.

Do you have any siblings? What are their names? How far apart in age are you? What was your relationship with them in your early years?
Describe the most formative experience(s) of your life for each of the following time periods: between ages 1-12, ages 13-19, ages 20-30. “Formative experiences” may include but are not limited to: births, deaths, weddings, divorces, traumatic events, anniversaries, graduations, and/or break-ups.

Describe the important events that take place in your life which occur during the course of the play. Why are these events significant? How are these events affected by the formative events that occurred earlier in your life?
